Benefits Available to EnviroTrac Employees

Medical Care Coverage
Medical Care Coverage is provided by Oxford Health Plans. We currently offer a High Deductible Health Plan (HDHP) and a Health Savings Account (HSA).  It is not a requirement to open an HSA account at the Exante Bank. You may choose a different bank that offers an HSA if you find it to be more convenient. You are eligible for benefits to begin the first of the month after 60 days of employment. Human Resources can provide you with any additional information about this benefit plan.

Dental Care Coverage
Dental Care Coverage is provided through Empire BCBS. You can look up provider listings on Empire Dental's web site. You are eligible for benefits to begin the first of the month after 60 days of employment. Human Resources can provide you with any additional information about this benefit plan.

401(k) Plan
401(k) Plan is offered through Oppenheimer Retirement Services. Your contributions can not exceed $15,500/year or 15% of your income, per IRS regulations. You are eligible to enroll in our 401(k) plan after 60 days of employment. If you choose to not enroll during your initial eligibility period you may enroll at a quarterly time frame thereafter.

Flexible Spending Account
Flexible Spending Account through ADP payroll service is offered for Medical Spending Account and Dependent Care Spending Account. The yearly maximum contribution for the medical account is $2,000.00 and for the dependent account is $5,000.00. These plans use pre-tax contributions to be applied to expenses related to non-covered medical or dependent care costs. If you elect to enroll in the Oxford HDHP and HSA, you are not eligible to enroll in an FSA.  More details are available at the following link: ADP's Flex Direct web site or through the HR Department. 15 Days of Paid Time Off (PTO) per Year
15 days of Paid Time Off (PTO) per year. PTO can be used in hourly increments for personal, sick and vacation time. Your time earned your first year is prorated. The 2nd to 5th years you can earn up to 15 days per year and after the 5th year of service you can earn up to 20 days per year. Each year there after full time employees are eligible to earn an additional day up to a maximum of 25 days per year (after the 10th year).Calculation stops at 40 hours worked per week.

Life Insurance and AD&D Insurance
Life Insurance and AD&D Insurance is provided by Reliance Standard Life Insurance Company. All full-time regular employees or part-time employees, who regularly work 30 hours a week or more, are eligible to participate in this plan after they have met the waiting period requirement. This benefit is available at no premium cost to the employee. In the event of the covered employee’s death the beneficiary will receive 3 times your earnings, up to a maximum of $400,000. Human Resources can provide you with any additional information about this benefit plan.

Short-Term Disability
Short-Term Disability is provided by Aflac. All full-time regular employees are eligible to participate in this plan after they have met the waiting period requirement. This benefit is individually customized to fit income and benefit needs. Aflac Short-Term Disability replaces your lost income, and benefits are paid regardless of any other insurance coverage. Deductions are made on an after-tax basis. Your Aflac Representative can provide you with any additional information about this benefit.

Personal Accident Indemnity Plan
Personal Accident Indemnity Plan is provided by Aflac. All full-time regular employees are eligible to participate in this plan after they have met the waiting period requirement. Aflac will pay cash directly to the employee for accidents that occur either on or off the job. Family coverage is available with this plan and deductions are made on a pre-tax basis. Your Aflac Representative can provide you with any additional information about this benefit.

Cancer Protection
Cancer Protection is provided by Aflac. All full-time regular employees are eligible to participate in this plan after they have met the waiting period requirement. Deductions are made on a pre-tax basis. This benefit helps to alleviate some of the financial burdens families incur when a family member becomes ill with cancer. Your Aflac Representative can provide you with any additional information about this benefit.
